Resumen

Resumen La Neopterina (NPT: Pteridin Trifostato de Guanosina) es liberada a la circulación por macrófagos activados por el interferón gamma (INF γ). En el presente estudio evaluamos el valor pronóstico de la Neopterina en el progreso de la infección por VIH-1 y su correlación con otros marcadores. Evaluamos el conteo de células CD4, carga viral por VIH y niveles de Neopterina en 108 muestras de sangre incluyendo 88 pacientes VIH+ (con SIDA y sin SIDA) y 20 sujetos VIH negativos. El conteo de CD4, la carga viral por VIH y los niveles de Neopterina fueron evaluados por medio de citometría de flujo, Amplicor HIV, Monitor Test 1.5 y por ELISA competitivo, respectivamente. La concentración promedio de Neopterina fue 5.7 nmol/L en el grupo control; sin embargo, en pacientes libres de SIDA sin tratamiento se encontraron altos niveles (45.8 nmol/L). Se presentó una fuerte correlación directa entre el índice CD4/CD8 y el índice CD4/NPT (r=0.973) en los pacientes con SIDA con TARV (terapia antiretroviral). Los índices CD4/NPT observados fueron 182.2 (IC: 149.1-215.4), 57.9 (IC: 39.3-77.6) y 5.2 (IC: 0.6-9.8) en el grupo control, pacientes con cargas virales bajas (<100,000 copias/ml) y pacientes con cargas virales altas (≥100,000 copias/ ml), respectivamente. Se obtuvieron diferencias estadísticamente significativas (p<0.0001) en las concentraciones de Neopterina en los pacientes con SIDA con respecto al tratamiento. Se encontró fuerte correlación directa (r=0.814) entre niveles de Neopterina y la carga viral en pacientes libres de SIDA y sin TARV. No obstante, se observó una correlación inversa entre los niveles de Neopterina y los conteos de CD4 (r =-0.485) en el mismo grupo de pacientes. Nuestros resultados demuestran el posible beneficio de la medición de Neopterina y el índice de CD4/NPT como marcadores pronósticos adicionales.

Neopterin and index value of CD4 / npt as prognostic markers of AIDS in HIV-positive patients

 

Summary

Summary neopterin (NPT: Guanosine triphosphate pteridine) is released into the circulation by activated by gamma interferon (INF γ) macrophages. In the present study we evaluated the prognostic value of Neopterin the progress of HIV-1 infection and its correlation with other markers. We evaluated CD4 cell count, HIV viral load and levels of Neopterin 108 blood samples including 88 HIV + patients (AIDS and non-AIDS) and 20 HIV-negative subjects.
